Healthcare & Referral Partners

Your patients need more than a diagnosis.

When a family receives a dementia diagnosis, the medical system has done its part — but the family's journey is just beginning. Tempus exists to meet families at that moment and walk alongside them. We partner with healthcare professionals to make that handoff seamless.

The gap after diagnosis

Most families leave a neurologist's office or a primary care appointment with a diagnosis and very little else. They don't know what to expect. They don't know what resources exist. They don't know who to call.

That gap between the clinical encounter and the support system is where families suffer most. It's where care partners burn out, where crises escalate, and where people fall through the cracks of a system that wasn't designed to hold them.

Tempus is designed to fill that gap. Our Family Compass navigation service, The Club adult day program, and Institute education programming work together to provide the wraparound support that families need — and that the medical system alone cannot provide.

What Tempus offers your patients

Family Compass navigation

A dedicated navigator who helps families understand the diagnosis, identify care options, access benefits and resources, and navigate the legal and financial planning that dementia requires.

The Club adult day program

A structured, therapeutic day program for adults living with cognitive decline — providing meaningful engagement, social connection, and professional supervision while giving family caregivers essential respite.

Caregiver education

Workshops and resources that help family caregivers understand the disease, develop caregiving skills, and sustain themselves through the long arc of the caregiving journey.

Caregiver support groups

Peer support groups for family caregivers — structured conversations with others who understand what they're carrying. Free to attend, open to any caregiver in Northeast Georgia.

Warm handoffs

We work with referring providers to make transitions smooth. A warm referral from your office means a family hears from a Tempus navigator within one business day.

Coordinated care communication

With family consent, Tempus navigators can communicate with referring providers about a family's care plan — keeping the clinical team informed and the care coordinated.

Who we work with

Tempus partners with a wide range of healthcare and social service professionals who encounter families affected by cognitive decline:

Neurologists and memory care specialists
Primary care physicians and geriatricians
